Accommodation Services
Accommodation Services is an active partner with students, faculty, and staff in creating an accessible learning environment where all students have an equal opportunity to participate in all aspects of Lakeshore College programs, services, and activities.
Accommodations are confidential and outcome neutral. They do not modify curriculum or provide any advantage or guarantee of success in a course or program.
STUDENTS WITH DISABILITIES
The American with Disabilities Act (ADA) of 1990 (42 U.S.C & 12101. et seq), the ADA Amendment Act of 2008, and Section 504(e) of the Rehabilitation Act of 1973 (29 U.S.C & 794), prohibits discrimination of persons because of their disability.
Accommodation Services works with students to develop an ADA Accommodation Plan.
- ADA Accommodation Plans are valid for one semester
- Students should establish ADA Accommodation Plans prior to the start of each semester
STUDENTS WHO ARE PREGNANT OR PARENTING
Title IX is a federal civil rights law that prohibits discrimination on the basis of sex in educational programs, services, and activities. This includes pregnancy and parenting status.
Pregnancy is a general term and is more broadly defined to include adoption, false pregnancy, abortion, and miscarriage.
Parenting is a general term and is more specifically defined to include situations involving a child or children diagnosed with a terminal illness, continuing treatment of a terminal illness, or death of a child.
- Title IX Accommodation Plans are valid for one semester
- Students should establish Title IX Accommodation Plans as soon as possible
TO REQUEST ACCOMMODATIONS:
- Submit an Accommodation Request Form
- Submit recent documentation (Note: documentation for disability should not be more than five years old)
- Documentation must identify disability or pregnant/parenting status
- Documentation must substantiate the need for accommodation
- Work with Accommodation Services staff to establish Accommodation Plan
DOCUMENTATION OPTIONS:
- Individual Education Plan (IEP) or 504 Plan – must include test results from re-evaluation conducted in high school
- Psychological Evaluation
- Medical Records or one of the following forms completed by a healthcare provider:
- Attention Deficit/Hyperactivity Disorder Documentation Form
- Autism Spectrum Disorder Documentation Form
- Blindness/Low Vision Documentation Form
- Deaf/Hard of Hearing Documentation Form
- Parenting Under Title IX Documentation Form
- Health, Systemic, or Mobility Related Disability Documentation Form
- Pregnancy/Childbirth Documentation Form
- Psychological Disorder Documentation Form
