Complaint/Concern Processes

Lakeshore Technical College provides opportunities for students, staff, and members of the community to file concerns and/or complaints below. The College will address and process submissions in a timely manner, according to defined procedures. Submissions will be reviewed during normal business hours. Submissions are not monitored after hours, on weekends, or during official college holidays.

Students, staff, or community members should use this feedback form to submit anonymous questions, concerns, suggestions, and/or requests at Lakeshore. This form is investigated by the appropriate department and used to make college improvements.

Non-Emergency Concern Regarding a Student

Students, staff, or community members should use this form to submit concerns related to student conduct (academic and non-academic), personal, or anything you feel is of concern. This form is reviewed by the CARES team.

Filing a Formal Complaint

Complainant is to complete & submit the appropriate online form below.  A formal complaint must include the basis for the complaint and the person or area to whom the complaint is about. 

Sexual Harassment Complaint

Students and staff should use this form to file complaints pertaining to possible violations of Lakeshore Technical College's Sexual Harassment Policy also referred to as Title IX . Examples of this type of complaint include gender-based discrimination, dating violence, domestic violence, hostile environment, sexual assault, sexual exploitation, stalking, retaliation, or other forms of sexual harassment. This form is reviewed by the Title IX Coordinator and the Title IX Student Deputy Coordinator as listed on Title IX policy.

Discrimination or Harassment Complaint

Students and staff should use this form to file complaints regarding potential discrimination or harassment that you have observed or been subjected to. Complaints submitted through this form will be reviewed by the Executive Director of Human Resources (Affirmative Action Officer) and the Vice President of Administration. Student involved complaints submitted through this form will involve the Access, Equity & Inclusion Manger (Equal Opportunity in Education Officer) and the Vice President of Student Success as well. The investigation will follow the appropriate procedure below:

Filing an Accident/Injury Report

Emergency – dial 911. If this is an emergency that involves imminent risk of harm to self or others, please contact emergency services by dialing 911. 

Accident/Injury for Non-Staff

Students and visitors should use this form to report any accident or injury suffered by a student or visitor (non-employee) while in any Lakeshore Technical College building, Lakeshore Technical College controlled property or Lakeshore Technical College clinical site. If possible, this report should be submitted within 24 hours of the accident/injury. This form is reviewed by the Director of Facilities.

Employees should use this form to report any accident or injury while in any Lakeshore Technical College building, Lakeshore Technical College controlled property or Lakeshore Technical College  clinical site. If possible, this report should be submitted within 24 hours of the accident/injury. This form is reviewed by Human Resources.

Education Conducted Across State Lines

Lakeshore Technical College participates in the National Council for State Authorization Reciprocity Agreement (NC-SARA). As a member of NC-SARA, any current or former student of a distance education program offered by Lakeshore Technical College may file a complaint with Wisconsin’s Distance Learning Authorization Board (DLAB) for Lakeshore Technical College distance education activity conducted across state lines under specific and limited circumstances.
